The directions below are excellent. The only detail I'd add is that the place to get your waiting number is the information window, which is the first window on your right as you enter the round office. There is a smaller office to the right with windows 15 and 16. That's not the spot.
All said and done, I got there at 10:30 on a Monday, and it took maybe an hour from the copy shop to the exit, including the time to fill out the form and make extra copies for the next border.
Also, if you have read anywhere that it takes 8 days, that's outdated information. It's a right then and there kind of thing.

I went today for visa renewal unfortunately, the visa extension for c4 countries got rejected as my passport was only valid for 5 months. Here is the step by step process to get it renewed:
WHERE: “Instituto Guatemalteco de Migración” located at JFCM+C9J, Ruta 4, Cdad. de Guatemala. This is in Zone 4 on Via 3 and Ruta 4 along 6a Avenida. Can see in ioverlander too.

WHEN: M-F, 8am-12pm (they are open until 2:30, but don’t bet on it). HOW: 5 Steps (below)

STEP 1 -print out the application form found online (Google: “APPLICATION FOR EXTENSION OF TOURIST OR TRAVELER VISA”) -fill out form -bring photo copy of passport (1) ID page AND (2) page with Guatemala (or other CA-4 country) entrance stamp -bring photo copy of (1) front and back of international credit card OR (2) your ticket leaving Guatemala (not need if credit card and you are overlander. If no credit card, see ioverlander note.) -bring Q200 in cash to pay

STEP 2 When you arrive, walk right into the entrance and tell security you’re doing a visa extension on floor 2. There will be various lines outside the building. They are for the bank and government offices located on other floors. The visa extension on floor 2 will not have a line outside. Go through the metal detector. Hand your passport to the check-in desk who will register your information.

STEP 3 Go up the stairs (there is an elevator) to floor 2. Walk into the office and wait in the short line for the first window if there is one. Walk up to the first window on the right hand side for information. Present your paperwork and they will give you a number. Sit and wait until your number is called (it’s automated and not very loud so listen carefully). When your number is called, go to the corresponding window and give them your paperwork; be polite and patient.

STEP 4 Once approved, the worker will give you a bill for the extension ($25 ~ Q192.50). You can only pay this bill downstairs at the bank on floor 1. And you can only pay in quetzales. Remember when I said one of the lines outside is for the bank? You have to leave the building and wait in this line (the one to the left when leaving the building). Once called, re-enter the building and go to the bank. Hand the teller your bill that you got from floor 2 and Q200. They will give you your change and a receipt.

STEP 5 Take your receipt from the bank and go back to the same exact window upstairs on floor 2 (no lines, no waiting). Give them your receipt, passport, and photocopies. They will then give you your 90-day extension stamp and you’re free to leave!!

NOTES/TIPS: -The extension is good for all CA-4 countries and is 90-days added onto your 90-day visa (180 in total). Meaning you could, in theory, fly into Guatemala City and the very next day get your extension so you can travel 6 months in the CA-4 without worry. -When looking at the entrance of the building, to the left is a photocopy shop. It’s only Q1 per photocopy. -Print out the visa extension application beforehand. To be honest, I did not and the lady working at the first window on Floor 2 printed it out for me for free. She did not look like she wanted to and did so very reluctantly, so I would not recommend this. -From start to finish the whole process took just under 1 hour. This included me not having photocopies ready and having to go get them next door. Note: I’m a white male from the USA, so the experience may not be the same for everyone.

Make first page passport copy and your guatamala stamp, print your Flight tickets, and fill the immigration form. 25$ for 90 days more.

And another thing to add.. written from the El Salvador/Honduras border, where I've just been refused entry into Honduras. You can 90 days issued, and that's fine for Guatemala and for El Salvador. It is not fine for Honduras! After 5 hours of arguing (in 40⁰ heat!) I accepted that it wasn't going to go my way this time, and am driving back to Mexico to get a fresh 90-day C-4 visa so I can make the 3-hr crossing through Honduras to Nicaragua!

The rationale they gave me is that Honduras issues just a 30-day extension; anything over this on your 90-day one issued in Guatemala is not valid there. I had been warned and decided to risk it; now I'm doing the warning. Buyer beware!

Just to add - if you're not travelling with a credit card, Expedia allow free cancellations of flights booked with them as long as it's within 24 hours of booking. Printout of their confirmation email worked for me!

Just extended, got 90 days BUT. Please note that if you don’t have a credit card (debit is not good enough) you’ll need a notarised document stating date of departure from C4 and proof of ownership of vehicle) you can also show a departure flight booking but for an overland traveler that is probably not going to happen. Lucky for us we found a lawyer nearby that got the paper sorted... the whole process took about 3hrs inc. lawyer visit... lawyer set us back Q300 and then $25 per passport for the extension.

Did my visa extension here with 3 days left on the CA-4 visa. Process is as described but took 3 hours in my case because it was busy. Best to go get the form and credit card photocopy from the shop round the corner before entering the building, to save some time. Was given 90 more days.

hice la prórroga del CA4 por más 90 dias. me solicitaron: copia del pasaporte, y copia de una tarjeta de crédito (frente y verso, borre el código atrás), saca la copia en la propia oficina por 1Qzl por página.
no necesita foto ni llevar el formulario, que vende a 1Qtz también en la copiadora.
pagas U$ 25.
Y recibe 30 min después tu nueva estampa.
yo lo hice con 20 dias antes del vencimiento, porque me iba al Interior de Guatemala.

we had to come to get the passport stamped as we were missing the entry stamp from guatemala in Melchor border. Fine of 200Q.
Better to come early as there are many people and a lot of queing. The office close at 2 but they dont accept reception of files after 12. To be paid in cash. Lot of patient!!!
