You need a copy of the certificate of health one copy of the vaccinations

If you have no copy, go right next door (looking at the doors it’s to the right) to the little tienda with no name. On the left there is a young man on a computer. He will enter you and your pet information online and give you a number plus make a copy of the health certificate. About 2.50 USD

Take that number and the copies to the VENTANILLA UNICA de comercio exterior. Give copies only. Pay $10 USD by card only. Wait. Get the official form.

I was told this is the very last step. There is nothing else needed

Report Check-In

1. You have to get you health certificate first (certificado de salud)
2. Make one copy of the health certificate as well as one copy of the pets vaccinations in the pets passport.
3. Go to the Edison Plaza into the Ventanilla única de comercio exterior (see attached picture)
4. The Person inside will give you the bank account number
5. Go to Banco Nacional and pay 10$ onto the account you received.
6. Go back to the Ventanilla and give all your paperwork and the bank payment to the agent. They will do the rest of the work.

as described the bank and the ventanilla de única are here. However before you pay and go to the ventanilla you have to fill in an online form.You can do this on siterpa.mida.gob.pa. The form is in English. Country of origin is Panama, not your home country. Fill in an address in Panama. Country of departure is where you are going to. Once filled in you get a number. You have to write down this number, you will not get an email, despite having to fill in an email address.

Step 2 of 3: After being at Andy’s Pets for our Heath Certificate & Co, we went to Plaza Edison (Info sheet from Vet) to Banco National to pay the us$5,- fee for the MIDA Export Document (“Licencia fitozoosanitaria de exportación”).

One of the bank employees helped me finding and filling in the correct form for the transaction - went to the line up for the counters (left when you walk in), payed and got the receipt.
Next: to an office called “Ventanilla Única” (also on the info sheet of vet) which is walking out of the bank straight ahead down the walkway...

PARKING: all parking at EdisonPlaza is paid parking and “fenced off” but possible to enter. We parked beside street, one stayed in the car, no costs.
